Mobile Health Education Modules (MHEd Modules) are an innovative digital tool providing short, accessible patient education sessions designed for specific health conditions. The MHEd Modules are delivered by Community Paramedics and are designed to help patients improve their health literacy, better understand their health condition, lifestyle factors, complications and how to manage symptoms.
Using an iPad during home visits, CPs guide patients through a series of interactive, easy-to-understand health education modules over several visits. Each module combines videos, visuals, and plain-language explanations tailored to different health literacy levels. The MHEd Modules also include printable resources, community paramedic teaching guides, and a patient workbook that encourages health learning, personal goal setting, and habit-building for sustainable behavior change.
Documentation can be integrated directly into your documentation software, allowing for seamless coordination across your CP team.
The MHEd Modules bridge gaps in access to specialized education by supporting patients who may be waiting for programs like cardiac or pulmonary rehabilitation. They also strengthen collaboration with primary care providers, ensuring patients receive consistent, personalized health education that complements their care plans.

How it Works:
The Community Paramedicine program registers for an annual team subscription to the MHEd Program
CP team members receive their login information and begin accessing the MHEd Modules. Modules include CP-only review of the topic designed to refresh key concepts.
The CP team flags patients who would benefit from the MHEd Program and the MHEd task is assigned to patient visits.
Prior to the visit, the CP can access printable resources from each module to bring to the appointment for the patient/caregiver.
The CP accesses the modules during the patient care appointments to deliver education over several visits on their tablet. Module features include videos, graphics, animations and resource guides
The CP can use teach-back prompts included in each module to reinforce patient learning
